STEP 1: COMPLETE THE WAIVER FORM

There is a fillable form below (Click here Now) or email admin@sheilahamilton.ca to request an electronic version of this waiver that you can sign and scan back to us.

STEP 2: COMPLETE THE GET ACTIVE SURVEY

This Questionnaire is required to be filled out please.

PLEASE NOTE; If you answer Yes to any of the questions in this Get Active Questionnaire then you will also need to print, complete and return form 1a below (GAQ Reference Document) to admin@sheilahamilton.ca.  


STEP 2a: FILL OUT THE GAQ REFERENCE DOCUMENT

You will only need to fill out the following GAQ Reference Document if you answered “yes” to any of questions in the Get Active Questionnaire in Step 1 above.


Otherwise, please skip this and proceed to Step 2

STEP 3: COMPLETE THE PERSONAL TRAINING LIFESTYLE QUESTIONNAIRE

Fill out this form to provide your trainer with more detailed information about your current health and lifestyle. This can assist the trainer in helping you make any lifestyle changes, getting you closer towards your health and fitness goals.

You might also want to fill out some of our other forms ahead of time, depending on your condition, goals, and needs.

DAILY FOOD AND MINDSET JOURNAL

If you would like to have a Precision Nutrition consultation, the Daily Food and Mindset Journal should be filled out for three separate days and emailed to sheila@sheilahamilton.ca at least one week prior to your consultation appointment. This allows Sheila to review your forms ahead of time, and make the most of your consultation time.

Hi everyone! I'm a first-year student at Corpus Christi. I'm currently studying for an art degree and taking a whole mixture of classes to figure out what I want to study in the future.


In the picture is my cat Lilo who I named after one of my favourite childhood movies, Lilo and Stitch (I have another cat named Stitch).


She always tries to sleep on my yoga mat while I'm exercising. I end up having to move her a couple of times before I can get started ha ha. 


I broke my leg quite badly last year and ending up having two surgeries. I lost a lot of my leg strength and mobility but since I started working with Sheila in the fall I've made a great recovery!


I've always wanted to have a regular workout schedule and plan and thanks to Sheila I've made that happen. 


I'm looking forward to becoming even stronger than before and getting back to doing hiking and camping trips.
